Georgia Licensed Care Manager
Job Summary
Providing ongoing remote care management, the full-time Georgia Licensed Care Manager will monitor patient vitals, develop individualized care plans, and collaborate with healthcare teams to enhance patient outcomes.
Key Responsibilities
- Monitor and interpret patient vitals, assessments, and alerts within the care management platform
- Develop, implement, and adjust individualized care plans addressing clinical and social needs
- Deliver patient education and coaching via telephone and secure messaging to empower chronic condition management
Required Qualifications
- Bachelor's degree in nursing or a closely related healthcare field
- Active and valid registered nurse (RN) license in good standing in the state of Georgia
- Approximately two (2) to five (5) years of direct patient care experience in Obstetrics and Gynecology (OB/GYN)
- Ability to work independently in a remote, technology-enabled environment
- Strong organizational skills with attention to detail and follow-through
