Job Summary

A company is looking for a Care Manager Transition of Care RN.

Key Responsibilities

• Evaluate member needs through post-discharge assessments and medication reconciliation
• Develop care/service plans and collaborate with healthcare teams to support member transitions
• Facilitate education and resources for members and coordinate services to meet their needs

Required Qualifications

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing
• 2 - 4 years of related experience in care management
• Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is required
• Must reside in the Atlanta, Georgia area
• RN Compact License or Multi-State License is a plus
