Job Summary

A company is looking for a Clinical Navigator - PT.

Key Responsibilities

• Conduct clinical intake assessments and gather information on patients' MSK conditions
• Build trust-based relationships and provide education to help patients understand treatment options
• Monitor patient progress and coordinate care between patients and healthcare teams

Required Qualifications

• Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required
• Current, unrestricted PT license in Georgia (Compact License preferred)
• Minimum 2 years of relevant experience in physical therapy or MSK-related roles
• Willingness to obtain additional licensure in new states as needed
• High comfort level with technology and data-informed clinical workflows
