Job Summary

A company is looking for a Clinical Navigator - PT (8am - 5pm MST).

Key Responsibilities

• Conduct outreach and educate patients on the program
• Perform intake assessments and provide health coaching throughout the care journey
• Collaborate with provider teams and ensure continuous updates and guidance for patients

Required Qualifications

• Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required
• Current, unrestricted PT license in Georgia (Compact License preferred)
• 3+ years of orthopedic experience
• Experience in motivational patient interviewing and coaching behavior change
• High level of comfort with technology, including Microsoft Office and CRM systems
