Job Summary

A company is looking for a Temporary Clinical Navigator - Part Time.

Key Responsibilities

• Conduct outreach to patients and educate them on the program's value
• Manage intake assessments and provide health coaching throughout the care journey
• Collaborate with provider teams and support programs to enhance patient experience

Required Qualifications

• Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required
• Current, unrestricted PT license in Georgia (Compact License preferred)
• 3+ years of orthopedic experience
• Experience in motivational patient interviewing and coaching behavior change
• Comfort with technology, including Microsoft Office and CRM systems
