Job Summary

A company is looking for a Temporary Clinical Navigator to provide personalized support to patients throughout their musculoskeletal care journey.

Key Responsibilities

• Conduct outreach to educate patients on the program and perform intake assessments
• Provide health coaching and condition education to support patient adherence to treatment plans
• Collaborate with provider teams and community support programs to enhance patient care

Required Qualifications

• Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required
• Current, unrestricted physical therapy license in Georgia (Compact License preferred)
• 3+ years of orthopedic experience
• Experience in motivational patient interviewing and coaching behavior change
• Strong comfort with technology and telephonic patient support environments
