Job Summary

A company is looking for a Temporary Clinical Navigator.

Key Responsibilities

• Conduct outreach to patients and educate them on the program's value
• Perform intake assessments to identify patient clinical issues and provide health coaching
• Collaborate with provider teams and support programs to enhance patient care

Required Qualifications

• Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required
• Current, unrestricted PT license in Georgia; Compact License preferred
• 3+ years of orthopedic experience
• Experience in motivational patient interviewing and coaching behavior change
• Proficiency with technology, including Microsoft Office and CRM systems
