Job Summary

A company is looking for a Clinical Navigator - PT.

Key Responsibilities

• Conduct outreach to educate patients about the program and perform intake assessments
• Provide health coaching and condition education to support treatment adherence and compliance
• Collaborate with care teams and community support programs to enhance patient experience and outcomes

Qualifications

• Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required
• Current, unrestricted PT license in Georgia; Compact License preferred
• 3+ years of orthopedic experience
• Experience in telephonic patient support and motivational interviewing
• High level of comfort with technology, including Microsoft Office and CRM systems
