Job Summary
Providing personalized support throughout the musculoskeletal care journey, the full-time Temporary Clinical Navigator will engage with patients and providers, manage intake processes, and facilitate care navigation while working remotely.
Key responsibilities
- Conduct outreach to educate patients on the TailorCare program and perform intake assessments to identify clinical needs
- Provide health coaching and condition education to support patient adherence to treatment plans
- Collaborate with care teams and community support programs to enhance patient experience and outcomes
Required qualifications
- Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required
- Current, unrestricted PT license in Georgia, Texas, and/or Colorado (Compact License preferred)
- 3+ years of orthopedic experience
- Experience in telephonic patient support and motivational interviewing
- Strong communication skills and adaptability in a fast-paced environment
