Job Summary
To provide personalized support throughout the musculoskeletal care journey, the part-time Georgia Licensed Clinical Navigator will manage patient intake, conduct outreach and assessments, and offer health coaching while working remotely.
Key responsibilities
- Conduct outreach to educate patients on the TailorCare program and perform intake assessments to identify clinical issues
- Provide ongoing health coaching and condition education to support patient adherence to treatment plans
- Collaborate with provider teams and community support programs to enhance the patient experience and ensure effective care navigation
Required qualifications
- Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required
- Current, unrestricted PT license in Georgia, Texas, and/or Colorado (Compact License preferred)
- 3+ years of orthopedic experience
- Experience in telephonic patient support and motivational interviewing
- High level of comfort with technology, including Microsoft Office and CRM systems
