Job Summary

A company is looking for a Clinical Pharmacist - Population Health - Remote.

Key Responsibilities

Lead initiatives to improve HEDIS medication adherence and pharmacy quality measures
Conduct Medication Therapy Management (MTM) sessions and educate patients on medication use
Monitor HEDIS performance metrics and prepare insights for leadership

Required Qualifications

Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
Active, unrestricted pharmacist license in Georgia (GA)
2+ years of experience in HEDIS medication adherence or population health
Strong knowledge of HEDIS measures and performance reporting tools
Experience with EHRs and pharmacy data analysis
