Job Summary

A company is looking for a Clinical Pharmacist - Population Health - Remote.

Key Responsibilities

Lead initiatives to improve HEDIS medication adherence and pharmacy quality measures
Conduct Medication Therapy Management (MTM) sessions to enhance patient adherence and medication regimens
Monitor HEDIS performance metrics and prepare insights for leadership to drive continuous improvement

Required Qualifications

Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
Active, unrestricted pharmacist license in Georgia (GA)
2+ years of experience in HEDIS medication adherence, quality improvement, or population health
Strong working knowledge of HEDIS measures, particularly medication adherence metrics
Experience with EHRs, pharmacy data, and performance reporting tools
