Job Summary

A company is looking for a Nurse Case Manager II - RN.

Key Responsibilities

• Utilize a collaborative process for assessment, planning, facilitation, and advocacy to meet individual health needs
• Conduct comprehensive assessments of referred members' needs and determine case resolution approaches
• Ensure compliance with regulatory and accreditation guidelines while administering benefits and utilizing case management processes

Required Qualifications, Training, and Education

• Active RN license in the state of residence
• 2-3 years of clinical practice experience in a healthcare setting
• 2 years of experience in the healthcare and/or managed care industry
• Case management experience is required
• Case Management Certification is preferred
