Job Summary
To support palliative care needs, the full-time remote Georgia Licensed Palliative Care LPN will conduct telephonic assessments, educate patients and families, and facilitate transitions to hospice care while collaborating with clinical teams.
Key responsibilities
- Conduct telephonic palliative assessments and determine next steps, including scheduling visits and arranging hospice referrals
- Complete hospice informational sessions with patients and families, providing education on hospice benefits and eligibility criteria
- Document in real time using EMR systems and support care transitions through accurate documentation
Required qualifications
- Current and unencumbered LPN license in Georgia
- 1+ years of hospice or palliative care experience in a patient-facing role
- Knowledge of Medicare hospice regulations and managed care
- Proficiency in Microsoft Office and general computer skills
- Experience in task triage and time management in a remote environment
