Job Summary
To support patient care needs, the full-time Georgia Licensed Palliative Care RN will conduct telephonic assessments, facilitate hospice transitions, and collaborate with clinical teams while working remotely from anywhere in the U.S.
Key responsibilities:
- Conduct telephonic palliative assessments and determine next steps for patient care
- Complete hospice informational sessions with patients and families, providing education on hospice benefits and eligibility
- Coordinate documentation to support referrals and transitions in care
Required qualifications:
- Current and unencumbered RN license in Georgia
- 1+ years of hospice or palliative care experience in a patient-facing role
- Experience supporting patients and families during end-of-life care
- Knowledge of Medicare hospice regulations and managed care
- Proficiency in Microsoft Office and general computer skills
