Georgia Licensed Palliative Care RN

Job Summary

To support patient care needs, the full-time Georgia Licensed Palliative Care RN will conduct telephonic assessments, facilitate hospice transitions, and collaborate with clinical teams while working remotely from anywhere in the U.S.

Key responsibilities:

• Conduct telephonic palliative assessments and determine next steps for patient care
• Complete hospice informational sessions with patients and families, providing education on hospice benefits and eligibility
• Coordinate documentation to support referrals and transitions in care

Required qualifications:

• Current and unencumbered RN license in Georgia
• 1+ years of hospice or palliative care experience in a patient-facing role
• Experience supporting patients and families during end-of-life care
• Knowledge of Medicare hospice regulations and managed care
• Proficiency in Microsoft Office and general computer skills
