Georgia Licensed Referral Manager

Job Summary

To support the admission process, the hybrid Referral Manager will screen referrals, coordinate services with providers, and manage the referral assessment process while ensuring timely and accurate completion of admission paperwork.

Key Responsibilities

• Utilize designated screening tools to complete referral screening and rescreening processes
• Oversee the statewide referral screening assessment process and maintain accurate referral statistics
• Coordinate admission paperwork and reconcile waitlists for timely completion and submission to the State of Georgia

Required Qualifications

• Nursing Degree from an accredited college or university, or graduation from an accredited/approved RN or LPN program
• Five years of nursing experience
• Current and unrestricted RN or LPN licensure in the state of practice
• CPR certification
• Valid driver's license and auto coverage
