Job Summary

A company is looking for a Registered Nurse (Remote Patient Monitoring and Chronic Care Management).

Key Responsibilities:

• Welcome patients into continuous care programs and review benefits and services
• Coordinate care with the patient's Respiratory Therapist and pulmonary providers
• Create personalized care plans and provide education on chronic pulmonary conditions

Required Qualifications:

• Active and Unrestricted Georgia RN License (Compact License preferred)
• At least three years of experience in adult health preferred
• Background in adult chronic health conditions (Pulmonology, Cardiology, Care Management) preferred
• Variety of Electronic Medical Record (EMR) experience
• Proficient knowledge and skills in basic computer usage
