Georgia Licensed RN Case Manager

Job Summary

To support managed care coordination, the full-time Georgia Licensed RN Case Manager will conduct comprehensive assessments, determine case resolutions, and collaborate with members to promote optimal health outcomes in a remote environment.

Key responsibilities

• Conduct comprehensive assessments of referred members' needs and eligibility using clinical tools and data review
• Determine appropriate case resolution approaches by evaluating members' benefit plans and available resources
• Collaborate with members and coordinate services to meet health needs and promote cost-effective outcomes

Required qualifications

• Active and unrestricted Georgia RN license or compact-state license
• Minimum of 3 years of clinical practice experience in hospital, home health, or ambulatory care settings
• Minimum of 3 years of case management experience
• Proficiency in computer skills, including navigating multiple systems
• High School Diploma or equivalent; Associate's Degree preferred
