Georgia Licensed Telehealth Physician

Job Summary

To support a mission-driven team, the full-time Georgia Licensed Telehealth Physician will provide routine telehealth visits with Medicare patients, manage personalized care plans, and collaborate with an integrated care team in a 100% remote environment.

Key responsibilities

• Conduct routine telehealth visits for E/M and Chronic Care Management with Medicare patients
• Create and manage personalized care plans for ongoing conditions
• Collaborate with nurse case managers, NPs, and Community Health Workers to ensure comprehensive patient care

Required qualifications

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Active medical license in Georgia; multi-state licensure is a plus
• Credentialed with major plans such as UHC, Aetna, and Humana
• Strong interest in preventive care, senior health, or chronic condition management
• Tech-savvy and comfortable practicing via telehealth
