Job Summary
Seeking a dedicated Georgia Licensed Telehealth RN, the part-time PRN Telehealth Medicare Annual Wellness Visit Specialist will conduct Medicare Annual Wellness Visits via telehealth, focusing on preventive healthcare and patient education in a remote setting.
Key responsibilities:
- Conduct Medicare Annual Wellness Visits via telehealth in accordance with CMS guidelines
- Review and update patient medical histories and assess current health status using remote monitoring tools
- Provide patient education on preventive measures and develop personalized prevention plans based on individual needs
Required qualifications:
- Current and unrestricted Registered Nurse (RN) license in the state of Georgia
- Minimum of 3 years of patient care nursing experience, preferably in primary care or telehealth
- Experience with Medicare Annual Wellness Visits (AWVs) and telehealth platforms is highly desirable
- Must be fully bilingual in Spanish and English
- Proficient in electronic health records (EHR) systems and telehealth technology
