Job Summary

A company is looking for a Telephonic Care Navigator.

Key Responsibilities
  • Completes telephonic outreach to members and educates them on gaps in care
  • Assists members with social determinants of health and connects them to community resources
  • Maintains schedule adherence and ensures compliance with HIPAA regulations
Required Qualifications
  • High School Diploma / GED or equivalent work experience
  • 1+ years of call center and/or telephonic customer service experience
  • 1+ years of professional experience in an office setting using telephone and computer
  • 1+ years of Healthcare/insurance or social work/community outreach experience
  • Ability to work full-time, Monday - Friday, and attend a 3-week training period
