Job Summary

To support a growing telehealth initiative, the full-time Geriatric Physician MD/DO Florida Licensed will conduct comprehensive assessments for older adults with chronic conditions, oversee care plans, and collaborate with healthcare advocates in a remote setting.

Key responsibilities

• Conduct comprehensive telehealth assessments for older adults with chronic and complex conditions
• Identify clinical risks and gaps in care across diagnoses, medications, and recent utilization
• Oversee patient care plans in partnership with healthcare advocates, addressing both medical needs and social determinants of health

Required qualifications

• MD or DO, licensed in at least 45 states including Florida, Maryland, Pennsylvania, and Texas
• Board certification in Internal Medicine, Family Medicine, or related specialty
• At least 5 hours of availability a week
• Experience caring for patients with chronic and complex conditions, especially older adults
• Strong clinical judgment and ability to synthesize incomplete information into clear plans
