Hawaii Licensed Disease Management Nurse

Job Summary

A company is looking for a Registered Nurse Disease Manager, remote with security clearance.

Key Responsibilities

• Perform assessments and coordinate care for dependents of Active Duty and eligible retirees
• Provide education and training related to disease management
• Work remotely while adhering to Hawaii residency and timezone requirements

Required Qualifications, Training, and Education

• Bachelor's Nursing Degree
• Hawaii state nursing license, with preference for additional compact state licensure
• At least 2 years of experience as a Registered Nurse Disease Manager, preferably in a remote setting
• Preferred certifications in disease management, such as Chronic Care Professional or Certified Diabetes Educator
