Job Summary

A company is looking for a Registered Nurse Disease Manager for a remote position.

Key Responsibilities

• Perform assessments and provide education and training to coordinate care for dependents of Active Duty and eligible retirees
• Work independently in a remote setting, managing a high volume of cases
• Utilize disease management certifications to enhance patient care and support

Required Qualifications, Training, and Education

• Bachelor's Nursing Degree
• Hawaii state nursing license; additional compact state license preferred
• At least 2 years of experience as a Registered Nurse Disease Manager, preferably in a remote environment
• Preferred certifications in disease management areas such as Chronic Care Professional or Certified Health Coach
