Hawaii Licensed Disease Management Nurse
Job Summary
To support the care coordination for dependents of Active Duty and eligible retirees, the short-term, fully remote Hawaii Licensed Disease Management Nurse will perform assessments, provide education and training, and manage care plans over an 8-week assignment that may extend further.
Key responsibilities
- Conduct assessments and develop care plans for patients
- Provide education and training to patients and their families regarding disease management
- Coordinate care and follow-up with healthcare providers and patients
Required qualifications
- Bachelor's Nursing Degree
- Current Hawaii state nursing license; additional compact state license preferred
- Minimum of 2 years' experience as a Registered Nurse Disease Manager, preferably in a remote setting
- Preferred certifications in disease management, such as CCP, CHC, CDE, or AE-C
