Job Summary

A company is looking for a Registered Nurse Disease Manager (RNDM).

Key Responsibilities

• Perform assessments, education, and training to coordinate care for dependents of Active Duty and eligible retirees
• Work remotely, managing a high-volume caseload effectively
• Collaborate with healthcare teams to implement disease management strategies

Required Qualifications

• Bachelor's Nursing Degree
• At least one active state nurse license, preferably compact state licensure
• Minimum of 2 years' experience as an RNDM, preferably in a remote or hybrid environment
• Current certification in a disease-management related area (e.g., CCP, CHC, CDE, AE-C)
• Access to secure, high-speed internet
