Hawaii Licensed Intake Coordinator

Job Summary

A company is looking for an Intake Coordinator - Social Worker.

Key Responsibilities

• Conduct initial screenings and collect demographic and clinical information from prospective patients
• Coordinate and schedule initial assessment appointments with clinical providers while maintaining patient records
• Provide support and guidance to patients throughout the intake process and advocate for their needs with external agencies

Required Qualifications

• Bachelor's degree in Social Work (BSW) or applicable Bachelor's degree; or Associate's degree in Social Work with a certificate in substance abuse treatment
• 1-3 years of post-licensure experience in case management or community-based social work, preferably in addiction or public health
• Current or license eligible, unrestricted Social Work license in the State of Hawaii (LBSW required)
• Strong knowledge of community resources and social service systems in Hawaii
• Proficiency or willingness to learn in using electronic health records (EHR) systems and telehealth platforms
