Hawaii Licensed LPN Care Coordinator

Job Summary

A company is looking for an LPN Care Coordinator to work remotely.

Key Responsibilities

• Coordinate transitions of care for TRICARE beneficiaries through inbound and outbound calls
• Communicate with community providers and ensure authorizations are in place to meet clinical care needs
• Document coordination activities accurately in the Medical Management System and participate in Quality Improvement Projects

Required Qualifications

• Must reside in Hawaii or Alaska
• Licensed Practical or Vocational Nurse (LPN/LVN) with a current unrestricted license
• 2+ years of clinical nursing experience, with a preference for 3 years
• 2+ years of Managed Care experience
• Proficiency in medical data entry and Microsoft Office
