Job Summary

A company is looking for a Licensed Practical Nurse (Bilingual/Spanish Speaking).

Key Responsibilities

• Support delivery of health care to patients by performing prescribed activities and procedures under the direction of Nurse Practitioners
• Provide one-on-one coaching and support to patients managing chronic conditions
• Conduct comprehensive assessments and monitor patients' progress through regular check-ins and remote tools

Required Qualifications, Training, and Education

• Bilingual/fluent Spanish speaker
• Active compact multi-state LPN/LVN License and HI state LPN license
• 5+ years of clinical experience as a Licensed Practical Nurse
• Experience in patient education for individuals managing chronic conditions
• Experience in supporting patients remotely is a plus
