Health Advisor

Job Summary

Providing care coordination and support for at-risk patients, the full-time Health Advisor will manage inbound and outbound telephonic outreach to promote patient engagement and improve health outcomes while working a hybrid schedule of 3 days remote and 2 days in-office.

Key responsibilities

• Assist physician practices in identifying and achieving quality care goals through direct patient outreach
• Collaborate with integrated care teams to support care coordination and ensure timely disease-specific and wellness visits
• Motivate patients to take proactive steps in their healthcare and facilitate access to appropriate care resources

Required qualifications

• Prior experience with Electronic Medical Record systems (e.g., Epic, Athena) and willingness to learn internal tools
• Background in patient outreach across diverse demographics, including transition of care and patient advocacy
• Ability to work a hybrid schedule (3 days remote, 2 days in-office)
• Comfort with navigating new platforms and using in-house digital tools
• Capability to evaluate complex patient cases and determine appropriate resources for escalation
