Health Advocate

Job Summary

A company is looking for a Health Advocate to support patients with chronic kidney disease and end-stage kidney disease.

Key Responsibilities

• Coordinate care with the clinical team and assist members with healthcare appointment scheduling and resource location
• Conduct outreach to inform members about the Kidney Services program and help them access community and government-based services
• Build rapport with members through communication and provide excellent customer service while documenting interactions accurately

Required Qualifications

• High School Diploma / GED
• Must be 18 years of age or older
• 2+ years of prior medical/healthcare experience in relevant fields
• Proficiency in Microsoft Word, Excel, and Outlook
• Ability to work 40 hours per week, Monday - Friday, with potential flex hours
