Job Summary

A company is looking for a Health Advocate on the Healthy Outcomes Team.

Key Responsibilities

• Conduct inbound and outbound calls to members and providers to facilitate healthcare services
• Address barriers to members' health and provide education for better health decisions
• Document conversations accurately in the member's health record

Required Qualifications

• High School Diploma or G.E.D
• High computer literacy, including troubleshooting skills
• Proficiency with Windows, web-based applications, and digital communication tools
• Reliable high-speed internet connection and suitable remote workspace
• Experience in a healthcare setting is preferred
