Job Summary

A company is looking for a Health Advocate.

Key Responsibilities

• Welcome and orient prospective patients to care management services
• Conduct high volume outbound/inbound calls to provide information about health plan programs
• Coordinate care delivery and maintain accurate documentation of consumer interactions

Required Qualifications

• High School Diploma/GED
• Must be 18 years of age or older
• 1 year of Case Management experience
• 2+ years of experience in the health care industry
• Experience with computers and Windows-based programs
