Job Summary

A company is looking for a Member Health Assessor.

Key Responsibilities

• Conduct health risk assessments and gather information from members and stakeholders
• Coordinate logistics to support care plan goals and assist with health education
• Maintain documentation and contribute to care plan development based on assessment outcomes

Required Qualifications

• High School Diploma or GED is required
• Minimum of two years of experience in community settings with at-risk populations is preferred
• Proficient in Microsoft Office applications
• Experience working with diverse cultures and communities
• Ability to work independently and collaboratively in a team environment
