Health Assessor

Job Summary

Engaging with members to establish effective relationships, the full-time hourly Member Health Assessor will conduct health risk assessments, coordinate care plan logistics, and provide health education while working remotely.

Key responsibilities

• Conduct health risk assessments by gathering information from members, families, and providers
• Coordinate logistics to support care plan goals and address immediate member concerns
• Contribute to the development and implementation of care plans based on assessment outcomes

Required qualifications

• High School Diploma or General Education Diploma (GED) is required
• Minimum of two years of experience working with at-risk populations in community settings is preferred
• Proficiency in Microsoft Office, including Outlook, Word, and Excel
• Experience working within diverse cultures and with sensitivity to their needs
• Ability to maintain confidentiality and demonstrate ethical conduct in all interactions
