Health Care Navigator

Job Summary

Engaging in proactive outreach, the full-time Health Care Navigator will build relationships with members and healthcare professionals, assisting members in navigating the healthcare system while working remotely.

Key responsibilities

• Conduct outreach to introduce and engage members with Population Health Management programs
• Collaborate with Care Teams to assess member needs and facilitate referrals to appropriate services
• Perform supportive tasks such as confirming member eligibility, scheduling appointments, and providing educational materials

Required qualifications

• Two years (60 credit hours) of college education or equivalent work experience
• Three years of experience in a clinical setting, human services, care management, or healthcare call center
• Ability to work as part of a multidisciplinary team
• Must reside in the Kansas City Metro Area
• High-speed internet service with specific performance standards for remote work
