Job Summary

A company is looking for an Analyst, Health Coach (Remote).

Key Responsibilities

• Educate members on managing health concerns and empower group coaching participation
• Assist members in collaborating with healthcare providers and care management teams
• Document member interactions and follow up on personal action plans

Required Qualifications, Training, and Education

• At least 2 years of experience as a Health Coach
• Experience in care management, healthcare, social work, or aging networks
• Associate's Degree required; Bachelor's preferred
• Health Coach Certification preferred
• Bilingual and community health experience preferred
