Job Summary

Empowering individuals managing chronic health conditions, the full-time remote Care Coach will guide members in setting health goals and developing actionable care plans while collaborating with an interdisciplinary team.

Key responsibilities

• Build trust and rapport with members to support their health management journey
• Educate members on health literacy and develop personalized coaching strategies
• Collaborate with healthcare professionals to optimize care and address barriers to treatment

Required qualifications

• Bachelor's degree in health education, nutrition, psychology, or related field
• Strong report writing and digital communication skills
• Certification or willingness to obtain certification from a nationally recognized coaching program
• Experience in behavioral coaching techniques such as motivational interviewing
• Excellent organization and time management skills
