Job Summary

To support a multi-disciplinary care team, the full-time Health Coach will engage with members through telephonic outreach, providing personalized coaching and education to help them achieve their health goals while working remotely.

Key responsibilities:

• Conduct inbound and outbound calls to engage and coach participants on behavior change and health improvement
• Coordinate care delivery with a multi-disciplinary team, ensuring effective communication and support for members
• Track member progress and program efficacy while adhering to schedules and operational requirements

Required qualifications:

• Bachelor's degree in a health-related field or equivalent certification
• 1-2 years of relevant experience, preferably in customer service
• Proficiency in Microsoft Office and data entry
• Ability to manage telephonic communication while navigating software platforms
• Knowledge of lifestyle-related topics such as nutrition, exercise, and stress reduction
