Health Coach

Job Summary

To support a multi-disciplinary care team, the full-time remote Health Coach will engage with members through structured telephonic outreach, providing education and motivation to help them achieve their health goals while adhering to established workflows and documentation standards.

Key responsibilities:

• Conduct engagement and coaching calls to inform and support program participants in behavior change
• Coordinate care delivery with team members, including nurses and pharmacists, to enhance member health outcomes
• Track member progress and participate in meetings to present findings and insights

Required qualifications:

• Bachelor's degree in a health-related field or NBHWC certification in lieu of degree
• 1-2 years of related experience, preferably in customer service
• Proficiency in Microsoft Office and data entry
• Ability to manage telephonic communication while navigating software platforms
• Knowledge of lifestyle-related topics such as nutrition, exercise, and stress reduction
