Health Coach

Job Summary

Seeking a mission-driven Care Coach to provide world-class care delivery services remotely, this part-time position will manage member caseloads, drive program engagement, and facilitate behavior change through personalized coaching and goal-setting.

Key responsibilities

• Drive program engagement by communicating the value of Maven's care coaching model and supporting long-term behavior change
• Co-create SMART goals with members, tracking adherence and adjusting care plans as needed
• Manage ongoing communication with members through messaging and video appointments while adhering to clinical guidelines

Required qualifications

• 1-2+ years of experience in health coaching or clinical work, focusing on patient education and motivational interviewing
• Experience working with maternity populations or individuals navigating family planning and fertility care
• Proven ability to achieve relevant KPIs related to patient outcomes, such as opt-in rates and member satisfaction
• Strong telehealth competency and comfort with technology tools like CRM platforms and EHRs
• Health coaching certification (NBC-HWC or ICF) is preferred but not required
