Job Summary

A company is looking for a Senior Analyst, Health Coach Wellness.

Key Responsibilities

• Conduct assessments of members' health status to promote medication adherence and health management
• Utilize motivational interviewing techniques to engage members and support behavior changes for optimal health
• Collaborate with members to identify and integrate available health programs and resources

Required Qualifications

• 1-3 years of experience with Microsoft Office
• 3-5 years of practice experience in relevant healthcare fields such as Renal, Diabetes, or Hypertension
• Experience in Managed Care and Medicare
• Associates Degree or equivalent experience
• Previous telephonic coaching or case management experience is preferred
