Job Summary

A company is looking for a Health Coach to support its Heart Failure Program remotely.

Key Responsibilities

• Provide patient education on chronic disease management, focusing on heart failure
• Establish and monitor patient goals through shared decision-making and follow up on patient communications
• Collaborate with the clinical team to ensure optimal patient outcomes and maintain patient relationships

Required Qualifications

• Health coach certification from an accredited program; a bachelor's degree in health sciences is preferred
• Experience in a medical office or healthcare environment, with cardiology coaching experience as a plus
• Experience with telehealth is a plus
• Proficiency in electronic health records and Microsoft Office applications is preferred
• Bi-lingual fluency in English and Spanish is highly preferred
