Job Summary

A company is looking for a Health Coach I (Remote) in Behavioral Health Integration.

Key Responsibilities

• Engages and supports medically vulnerable or chronically ill patients in their health journey
• Implements coaching techniques and facilitates referrals to resources and services
• Provides customized education on health management and collaborates with the primary care team

Required Qualifications

• High School diploma or equivalent required
• Health Coach Certification required (or to be obtained within 1-2 years of employment)
• Preferred: Bachelor's degree in a health discipline
• Ability to lift and support weights up to 50 pounds
• Must have a full range of sensory abilities and mobility to travel to various locations
