Job Summary

A company is looking for a Health Coach - Clinical Navigator.

Key Responsibilities

• Provide one-on-one health coaching for patients managing chronic conditions
• Use motivational interviewing and behavioral change techniques to guide patients
• Educate patients on disease management, medication adherence, and preventive care

Required Qualifications

• Active health coaching certification (e.g., NBHWC, NSHC, ACE Advanced)
• 1+ years of experience in health coaching or patient support
• Proven expertise in behavior change techniques and motivational interviewing
• Experience with remote patient care or telehealth is highly desirable
• Strong organizational skills and ability to meet deadlines in a fast-paced environment
