Job Summary

A company is looking for a Health Coach I to engage and support patients in their health journey.

Key Responsibilities

• Collaborates with patients to actively engage them in their health care and provide support and education
• Facilitates referrals to services and resources, advocating for patients' needs
• Communicates care plans and coordinates care across various health settings

Required Qualifications

• High School diploma or equivalent required; Bachelor's degree in health discipline preferred
• Health Coach Certification required or to be attained within one to two years of employment
• Ability to work in various settings including office, clinic, and community environments
• Must be able to lift and support the weight of 50 pounds
• Requires intact sense of sight, hearing, smell, touch, and finger dexterity
