Job Summary

A company is looking for a Full Time Health Coach I - Remote.

Key Responsibilities

• Establish trusting relationships with patients and support them in managing their health
• Communicate effectively with patients across various platforms and coordinate care with the healthcare team
• Provide education on chronic disease management and assist with care coordination

Required Qualifications

• Health Coach certification is preferred
• Prior experience in a service environment is required
• Some clinical experience is preferred (e.g., EMT, MA, or CNA)
• Ability to work collaboratively in a team setting
• Familiarity with multiple non-integrated platforms is necessary
