A company is looking for a Professional Health Coach in Digital Medicine, working remotely.

Key Responsibilities

• Conducts outreach calls with patients to support their participation in the digital medicine program
• Collaborates with clinical staff to coordinate patient information and manage chronic conditions
• Provides education and encouragement to patients regarding health interventions and lifestyle changes

Required Qualifications

• High school diploma or equivalent
• 5 years of patient care experience, or a bachelor's degree in a related field with no experience required
• Certification for MA, LPN, or Pharmacy Tech program, or no certification required with an associate's degree in a related field
• Preferred certifications include Certified Health Education Specialist (CHES) or accredited health coaching certification
• Proficiency in using computers, software, and web-based applications
