Job Summary

A company is looking for a Health Coach Manager to support care teams in achieving performance goals and enhancing patient outcomes.

Key Responsibilities

• Manage and mentor Health Coaches, overseeing their onboarding and ongoing performance
• Facilitate weekly meetings to discuss team successes and challenges, fostering a collaborative environment
• Monitor performance metrics and conduct quality assurance calls to ensure effective patient engagement

Required Qualifications

• 3+ years of supervisory experience in a relevant field
• Experience in managing people and guiding performance
• Familiarity with social determinants of health
• Experience working with diverse and remote teams
• Knowledge of primary care, health coaching, or care for vulnerable populations is preferred
