Job Summary

A company is looking for a Health Coach (NBC-HWC).

Key Responsibilities

• Provide one-on-one coaching to members managing or at risk for chronic health conditions
• Use evidence-based guidelines to educate and empower members in achieving health goals
• Partner with healthcare teams to improve member experience and outcomes while reducing costs

Required Qualifications, Training, and Education

• Associate degree, preferably in a healthcare-related field
• Completion of a training program approved by the National Board for Health and Wellness Coaching (NBHWC)
• At least one year of experience in a health coaching-related field preferred
• Experience with chronic conditions such as diabetes, obesity, and hypertension is preferred
• Solid understanding of health behavior change theories and their practical application
